New Student Enrollment

Thank you for allowing us to partner with your family. It’s an honor to be part of your journey, and we look forward to a joyful and meaningful experience together.
FinalForms is our district’s online enrollment and forms system. Families can use FinalForms to complete registration, update contact information, sign required documents, and keep important student information current in one secure place. The system saves your information from year to year, making future updates faster and easier.
If you are a resident of Holt/Dimondale or have been selected as a School of Choice family for the 2026-2027 school year, please follow these directions to complete your student's enrollment. Remember to select the 2026-2027 school year.

Immunizations, Dental, Hearing & Vision Requirements
Annual Immunization or Waiver Information:
An immunization record is required for enrollment. Any parent/guardian wishing to claim a non-medical waiver must receive education regarding the benefits of vaccination and the risks of disease from a county health department before obtaining the certified non-medical waiver form from the local health department. The rule requires using the State of Michigan form dated for the current calendar year. For more information, contact the Ingham County Health Department at 517.887.4350.
